In Italy, Denture Wearers Almost Never Use Adhesive. Here Is Why American Dentists Won't Tell You.

Why "use more cream" was never going to work — and the material Italian clinics have used for decades instead

Denture cream comparison

In Italy, denture wearers almost never use adhesive cream. It is not the standard. It never has been.

We spend appointments talking patients out of it.

I moved to America two years ago when my husband accepted a position at a hospital in Chicago. I joined a practice near the North Side. Good practice. Good people.

And in my first few months in the exam chair, I saw something I could not believe.

Patient after patient sitting across from me with the same look on their face.

Ashamed. Worn out.

Every one of them had been told the same two things:

"Use a stronger adhesive."

"Or start saving for implants."

Nobody wants to reapply paste three or four times a day. Nobody wants to excuse themselves from a dinner table to fix their mouth in a restaurant bathroom. And nobody should have to scrub residue off their gums for 15 minutes every night and go to bed still tasting it.

But here is the part that disturbed me most.

It never gets better. The seal only gets weaker. Every year the denture holds less. Every year the reapplications happen more often. And nobody in the American system explains why.

That is why in Italy we do not hand people a tube of paste when their denture stops holding. We address why it stopped holding in the first place.

What Your Dentist Should Have Told You the Day Your Teeth Came Out

Dr. Brenda Sue explaining the gap mechanism

The day your teeth were extracted, your jawbone started to resorb.

The horseshoe of bone your denture sits on shrinks a little every month. It never stops. Not in year one, not in year five, not ever.

Your denture was carved for the ridge you had the day it was made. The ridge underneath has been shrinking ever since.

That means there is a physical gap between the plate and the ridge now. Small enough that you cannot see it or feel it. Big enough that the denture floats every time you eat.

And adhesive cannot fill that gap.

Adhesive is a bonding agent. It sticks two surfaces that are already touching. It cannot cross a space, no matter what brand you buy, no matter how much you apply.

In plain language: paste is glue. Glue needs both surfaces pressed together. Once there is air between them, glue has nothing to hold onto.

That gap you cannot see is why every tube on your bathroom counter has failed you.

What American Dentists Already Know (And Do Not Tell You)

Patient consulting her dentist

In my first month at the practice in Chicago, I sat down with my American colleagues and described what I had been seeing.

I said, "You know why the cream fails, right? It is not the brand."

They knew. Every one of them knew about resorption. Every one of them understood that the gap opens and the paste cannot cross it.

And not one of them had ever explained it to a patient.

Because explaining it would mean admitting that the tube they just recommended cannot do what they implied it would do.

"Patient reapplying 4x daily. Cream residue causing tissue irritation. The gap is the problem. But you try telling a patient that after you just sold them a tube."

"Every denture wearer in my chair blames themselves. Every one. And nobody corrects them because the correction leads to a conversation nobody wants to have."

"Gave a patient thermoplastic for the first time last month. She called me 3 days later. Said she ate a steak. First one in 5 years."

"In Italy this is standard. Here it is not even mentioned. That should bother every one of us."

What We Use in Italy Instead of Cream

Denture adhesive comparison chart

In Italian dental schools we are taught that one specific class of material can fill that gap and do what adhesive cannot.

Dental-grade thermoplastic.

It is a solid material at mouth temperature. It softens when warmed and molds into the exact shape of whatever it is pressed against. It holds that shape when it cools.

We use it in Italy for anything in dentistry that has to fit one specific mouth and stay exactly where it is placed.

For decades, prosthodontic clinics across Italy have used a gel form of this same material to refit loose dentures. You warm the tube. Press it into the denture. Bite down. The gel takes the shape of the exact gap that formed as your bone resorbed. The denture finds the ridge again.

"That is not adhesion. That is shape. And once you understand the difference, adhesive stops making any sense at all."

Why Nobody in America Told You

Denture cream shelf

When I started looking for this material in American pharmacies, I could not find it.

What I could find were tubes labelled "thermoplastic gel" sold cheap on Amazon. I looked at them the way I would look at a knockoff.

Most of them were off-the-shelf polymer. The same cheap plastic used in moldable toys. Softens fast, hardens fast, no memory, no give. Wrong material for the job.

The gel we use in Italy is dental-grade. The same material we use in the lab for prosthetics. It costs more to make. It behaves differently. It holds for days at a time, not hours.

And the reason it is not on the shelf at your pharmacy has nothing to do with science. A tube that quits by lunch gets bought again in two weeks. A material that holds for days does not.

You were never doing it wrong. You were never told there was something better.

The Brand Italian Clinics Use

The product I use in my practice is called Oragel. Made in Italy. Same dental-grade thermoplastic I was trained on for over two decades.

Here is what makes Oragel different:

Dental-Grade Thermoplastic The same class of material used in Italian clinics for decades. Not the consumer polymer sold cheap online.
Fills the Gap, Does Not Coat It Molds to the exact shape of the space bone loss opened up. The plate sits flush again.
Not Water Soluble Saliva cannot break it down. That is why it holds for days instead of hours.
Zero Residue Peels out in one piece. No scrubbing. No 15 minutes at the sink.
Works on the Lower Plate The one every cream fails on first.
Oragel comfort that lasts days

What Happened When I Gave It to My First Patient Here

Her name was Sofia. She was 68.

For three years Sofia had been telling me the same thing. Her denture held fine sitting in my office. The second she started eating, it moved.

She had not eaten a proper meal in front of anyone outside her home in over two years.

She hosted her family every Sunday for dinner. Four generations at her table. She was the one who plated everyone's food and then sat at the head talking while they ate. She would eat in the kitchen afterwards. Alone. Yogurt. Soup. Bread torn into pieces small enough that the denture would not shift.

I showed her how to warm the tube in a mug of hot water. Two lines of gel into the denture. Press in, bite down, trim the extra.

I told her to try it before Sunday dinner that week.

Day 1: "Brenda, I Ate."

She called me on Monday morning. She had roasted a whole chicken for the family. Potatoes. A salad. Bread with the good olive oil.

She sat down at the table with everyone else for the first time in over two years. She ate the chicken. All of it. And a second helping of the potatoes.

"Nothing moved. The whole meal. Nothing moved."

Day 14: She Forgot She Had Dentures

"It has been fourteen days and I have not touched the tube of glue once. I have not reapplied in the middle of a meal. I have not carried it in my purse. I forgot I even had it."

Day 21: The Anniversary

Her husband and I went out for their forty-fifth anniversary. They went back to the restaurant where they had their first anniversary. She ordered the osso buco. She ate the osso buco. She had two glasses of wine and gave a speech at the end.

Her daughter told her afterwards she had not seen her mother talk that much in three years.

Day 28: "I Could Not Live Without It."

Sofia came into my office for a checkup. I asked her the question I ask every patient I have put on the gel.

"Do you still need it?"

She looked at me like the question offended her.

"Brenda, I feel like myself again. I could not live without it."

What denture wearers are saying

⚠️ Important: Don't Buy the Cheap Version

Marketplace versions vs Oragel

Please do not go buy any "thermoplastic gel" off Amazon.

Almost every tube sold there is the wrong material. Consumer-grade polymer that sets uneven and will make you think thermoplastic does not work, when the truth is you were given the wrong grade by someone who does not know the difference.

The gel we use in Italian clinics costs more to produce because it behaves differently. It molds precisely. It seals at the edges. It holds for days because saliva cannot dissolve it.

Oragel is the only brand made in Italy from the same dental-grade thermoplastic I was trained on. It is not sold on Amazon or in pharmacies. Only through the official site.

Two Paths Forward

Before and after

Path 1: You close this page.

Next week you are still reapplying at lunch. Still cutting food small. Still eating before you go out so you do not have to eat there.

Six months from now the gap is a little wider. The cream holds a little less. The dinners with friends are one more excuse away from gone.

Path 2: You fill the gap.

A few minutes at the same sink you already stand at every morning.

This week you sit through a meal without checking. You order what you actually want. You stop carrying a tube in your purse.

Six months from now the cream aisle at the pharmacy is a place you walk past without stopping. And Sunday dinner is yours again.

I am not writing this to sell you something. I am writing this because I moved to a country where millions of people are using a product that physically cannot do the job they need it to do, and not one person in the system that sells it to them has the incentive to say so.

In Italy we would call that a failure of care.

The choice is yours.
